Most failed SharePoint implementations we are called into were migrations. Somebody copied a file server that had grown organically for a decade - nested folders eight deep, three versions of every document, permissions nobody could explain - directly into SharePoint. The structure arrived intact, staff could not find anything, and within months everyone was emailing attachments again.

Fixing the structure before moving the data is the difference. So is planning for change: permissions that made sense at the outset need to still make sense after your third staff turnover, and that only happens if they were designed rather than inherited.
